This Privacy Policy explains how Junction Health Inc ("Junction Health," "we," "us," or "our") collects, uses, and protects personal information in connection with our website https://junction-health.com (the "Site"), our text messaging program, and our related communications and services.

We are committed to protecting the privacy of the people we work with, including patients, referring providers, business partners, and visitors to our Site. We do not sell or rent your personal information, and we do not share your mobile phone number or other mobile contact information with third parties for their marketing purposes.

1. Scope of this Policy

This Policy applies to personal information we collect through our Site, our email and text (SMS/MMS) communications, and our routine business operations. It does not cover information that is protected health information ("PHI") governed by the Health Insurance Portability and Accountability Act ("HIPAA"). Our text messaging program is intended for general, administrative, and coordination communications and is not intended for the exchange of protected health information. Please do not send sensitive health details by text.

2. Information We Collect

We collect the following categories of information:

Information you provide directly. Your name, business or personal contact information (including email address, mailing address, and mobile or landline phone number), the organization or practice you are affiliated with, your role, and the contents of messages, forms, or requests you submit to us.

Information collected automatically on our Site. When you visit the Site, we may automatically collect technical information such as your IP address, browser type, device information, pages viewed, referring pages, and the dates and times of your visits. We may collect this information using cookies and similar technologies (see Section 8).

Text messaging information. If you opt in to our text messaging program, we collect your mobile phone number, your consent record, and the content and metadata of the messages exchanged (such as timestamps and delivery status).

We do not knowingly collect personal information from children under 13.

3. How We Use Your Information

We use personal information to:

  • respond to your inquiries, requests, and referrals;

  • coordinate care logistics and communicate with referring providers, partners, and their staff;

  • send text messages you have consented to receive, including scheduling, coordination, status updates, and account or service notifications;

  • operate, maintain, secure, and improve our Site and services;

  • send administrative and, where permitted, informational communications;

  • comply with legal, regulatory, and contractual obligations; and

  • protect the rights, property, and safety of Junction Health, our patients, and others, and to prevent fraud or misuse.

We process personal information based on your consent, our legitimate business interests, the performance of a contract or service, and our legal obligations, as applicable.

4. Text Messaging (SMS/MMS) Program

This section describes our text messaging program and applies to everyone who opts in to receive text messages from us, including referring providers, their staff, business partners, and, where applicable, our own team members.

Consent and opt-in. By providing your mobile phone number and opting in, you agree to receive text messages from Junction Health at the number provided. Consent to receive text messages is not a condition of receiving any service from us. Message frequency varies.

Program purpose. Messages are used for administrative and coordination purposes, such as communicating with referring providers and their staff, scheduling and logistics, status updates, and related account or service notifications. Our provider relationship managers may exchange text messages with referring providers to coordinate day-to-day communications.

Costs. Message and data rates may apply according to the plan you have with your wireless carrier. Junction Health is not responsible for these charges.

Opting out. You can cancel the text messaging program at any time by replying STOP to any message you receive from us. After you reply STOP, we will send a one-time confirmation and will not send you any further text messages unless you opt in again. You may also contact us using the details in Section 12 to opt out.

Carriers. Wireless carriers are not liable for delayed or undelivered messages.

No sharing or selling of mobile information for marketing. We do not share, sell, rent, or otherwise disclose your mobile phone number or text messaging consent to any third party for that third party's marketing purposes. Mobile information collected through our text messaging program is used only to operate and support the program as described in this Policy. We may share this information with service providers that help us operate the messaging program (for example, our messaging platform or carrier), subject to the limits in Section 5, and only to the extent necessary to deliver the messages; we do not authorize these providers to use the information for their own marketing.

5. How We Share Information

We share personal information only as described below. We do not sell your personal information, and we do not share mobile contact information with third parties for their own marketing.

Service providers. We share information with vendors and service providers who perform services on our behalf — such as website hosting, IT and security, communications and messaging platforms, and analytics — under contracts that require them to protect the information and to use it only to provide services to us.

Affiliates and business partners. We may share information with our affiliated entities and with partners solely to provide, coordinate, and support the services you have requested or that we jointly provide.

Legal and safety. We may disclose information when required by law, regulation, legal process, or governmental request, or when we believe disclosure is necessary to protect our rights, enforce our terms, prevent fraud, or protect the safety of any person.

Business transfers. If we are involved in a merger, acquisition, financing, or sale of assets, information may be transferred as part of that transaction, subject to this Policy.

6. How We Protect Your Information

We maintain reasonable administrative, technical, and physical safeguards designed to protect personal information against unauthorized access, use, alteration, and destruction. No method of transmission or storage is completely secure, however, and we cannot guarantee absolute security.

When information is no longer needed, we take reasonable steps to delete or de-identify it.

You can control cookies through your browser settings; disabling certain cookies may affect Site functionality.   • opt out of promotional emails by using the unsubscribe link or contacting us;

  • request access to, correction of, or deletion of the personal information we hold about you; and

  • ask questions about how your information is used.

Depending on where you live, you may have additional rights under applicable privacy laws (such as the California Consumer Privacy Act, as amended). To exercise any of these rights, contact us using the details in Section 12. We will respond as required by applicable law. We will not discriminate against you for exercising your privacy rights.

11. Changes to this Policy

We may update this Policy from time to time. When we do, we will revise the "Last updated" date above and post the current version on this page. Material changes will be communicated as required by law. Your continued use of our Site or services after an update means you accept the revised Policy.
